Birth Announcement Form

The Courier Express will post your birth announcement to our web site. In order to do so please download our birth announcement form (PDF) below. The form cannot be filled out online but will need to be printed out, filled out and either brought in or mailed in to us. Please note that we will need signatures for the "Who's the Daddy?" portion.

Parents or surrogates should submit a completed form to one of the following offices: The Courier Express/Tri-County Sunday in DuBois; The Jeffersonian Democrat in Brookville; or the Leader Vindicator in New Bethlehem.
